Background Release Form
At OCU, the safety of our campus is paramount. Before you are able to begin work at OCU, you must complete the background release form. Your employment is contingent upon the results of your screening.
Please contact all of your degree-granting institutions and request that your official transcripts be sent directly to your dean's office (we cannot accept a transcript from any source other than the degree-granting institution). The mailing address for each academic unit is 2501 N Blackwelder Ave, Oklahoma City, OK 73106.
Form I-9, Employment Eligibility Verification
Oklahoma City University is required by law to complete an Employment Verification Form (Form I-9) for all employees. A new employee must complete an I-9 Form no later than close of business on his/her first day of work. Please note that the federal government may possibly impose civil penalties on OCU when a new employee has not completed his/her I-9 by the close of business on their first day of work.
You must visit Human Resources to complete an I-9 Form. Please review the form for a list of eligible documents. Original documents are required. Office hours are 8:00am to 5:00pm, Monday - Friday. Direct Deposit Authorization Form
Direct Deposit is mandatory for all university employees. A voided check should be attached to the direct deposit form.
Confidentiality and Privacy Statement
Please complete and sign the Oklahoma City University Confidentiality and Privacy Statement.
Social Security Card
As a matter of payroll administration, it is necessary for every employee to provide a valid, signed Social Security card at the time of hire. This is a condition of employment and is not optional. If you cannot provide your card at the time of hire, you must immediately request a replacement card. The closest Social Security Administration office is located at Shepherd Mall, 2615 N Villa, Oklahoma City, OK 73107, with hours from 9am until 4pm. We will accept your receipt in lieu of your replacement card to establish a temporary payroll record. You are required to present your new card to Human Resources as soon as it arrives.
Sexual Harassment Training Certification
Human Resources will contact you at the e-mail address indicated on your Employee Information Sheet with instructions for completing your training.
TIAA-CREF Supplemental Retirement Plan

Return completed documents to Human Resources. Your payroll will not be processed until you have returned all of the required documents and satisfactorily passed the background screening.
Once your paperwork is processed by HR, you will receive your "B Number" from your department contact. This number will serve as your campus-wide ID.
Within your first week, please visit Student Account Services on the 3rd floor of the Clara E. Jones Administration Building between 8am and 5pm to receive your ID. You may contact Student Account Services at (405) 208-5146 if you have questions about your ID.
Parking permits are obtained from the Oklahoma City University Police Department, 3rd floor, Clara E. Jones Administration Building. Permits cannot be obtained without an OCU photo ID. Employees are charged monthly. You may elect to have your parking permit fee payroll deducted. Please have your vehicle license plate number available when applying for your parking permit.
