Affidavit of Identity and Statement of Education Purpose (.pdf)

This form may be requested as part of the verification process. Student must appear in person at the Financial Aid Office with required identification to submit and sign the form. If student cannot appear in person follow the specific instruction for submitting the form with a Notary’s Certificate of Acknowledgment.

Authorization to Release Financial Information (FERPA) (.pdf)

Student can complete the FERPA form to authorization the Financial Aid office to release financial information to parents and/or spouse. Student must submit this form online through BlueLink.

Direct Deposit - Parent Plus (.pdf)

All refunds are made by direct deposit in to personal checking/or savings accounts. We must have a direct deposit form on file for any Parent that has a Parent Plus Loan. This will ensure that if there is a refund of the Parent Plus loan the funds will be sent to the correct account.

Emergency Advance Application (.pdf)

On the first day of class a student can submit the application to request an advance of any pending refund of excess aid for the current semester. The amount is limited to $400. Student must submit form in person to the Financial Aid Office for review.

Parent Resource Letter (.pdf)

Form is requested when parent(s) report unusually low income on the FAFSA. The information will be used to confirm living arrangements and ensure accuracy of your application. Student and parent must sign the form and submit to the Financial Aid Office.

Red Earth Advance Application (.pdf)

Application for a financial aid advance for students in the Red Earth MFA Program only.

Request for Additional Financial Aid for Education-Related Expenses (.pdf)

If a student has additional education related expenses that result from attending OCU they may request additional aid. If approved additional Parent Plus loan or Graduate Plus loans can be offered. Submit completed form with detailed supporting documentation and or receipts to the Financial Aid Office for review.

Satisfactory Academic Progress Policy (.pdf)

A student must demonstrate Satisfactory Academic Progress (SAP) to receive financial aid. The SAP Policy defines the completion rate and grade point average required to remain eligible for financial aid.

Verification Worksheet for Dependent Students (.pdf)

Verification is a process to review the accuracy of the information reported on your FAFSA by comparing it with your and your parent(s) Federal income tax information and/or other financial documents. Submit the completed form signed by you and a parent with any additional documentation requested to the Financial Aid Office.

To complete your Perkins exit counseling, please contact the Oklahoma City University Student Accounts office at [email protected] and you will be sent instructions on completing the online exit counseling through our loan servicer, UNISA, Inc. Once you complete your Perkins Exit Counseling online, please read the Perkins Information Packet below. This packet will provide vital information regarding your Perkins lender, where to send payments, consequences if you default and your responsibilities regarding the loan. After all documents have been reviewed, sign, date, and return the last page of the packet — Perkins Information Acknowledgement — to the Student Accounts Office.

Completing this requirement will finalize your Perkins Exit and the Perkins hold will be released from your Student Account.

If you are entering one of the fields listed below, you may qualify for loan cancellation benefits. For your convenience, the cancellation information is available to you at Loan Forgiveness or you can schedule an appointment with the Perkins Loan Counselor by e-mailing [email protected].

  • Full-time Teacher - in a low-income public or nonprofit elementary or secondary school
  • Full-time Early Intervention Services
  • Full-time Law Enforcement
  • Full-time Nurse or Medical Technician
  • Full-time Child or Family Services
  • Head Start
  • Military serving in hostile areas
  • Full-time Volunteer Services
  • Full time staff in pre-kindergarten or child care program licensed by the state
  • Full-time lawyers employed by public defender or district attorney
  • Full-time firefighters
  • Full-time faculty members of Tribal college or university
  • Librarians with a Master's Degree in Library Science employed in low-income schools
  • Full-time speech language pathologists with a Master's degree working with low-income children
