What Do I Need to Have?

All students regardless of year of study (undergraduate, graduate and law students) or where they live are required by Oklahoma Statute to provide documentation of having the following vaccinations:

  • Hepatitis B - a series of 3 injections given over a 6 month period. 
  • MMR (Measles, Mumps, Rubella)- usually given to children before elementary school and there are 2 injections.

All students living in the dormitories on campus are required by Oklahoma Statute to provide documentation of vaccination against Meningococcal disease or to sign a waiver declining this vaccine. 

All students living in on-campus housing (regardless of year of study) are required by OCU to either get the Meningococcal vaccination or sign a waiver against the vaccine.  The meningitis vaccine is available for purchase during walk-in hours in the Campus Health Clinic.

How Do I Turn in Records?

  • No original records will be accepted.  Make a copy of the immunization record for Campus Health and keep the original in a safe location for your records.  Write your name, Star Access ID Number (B number), and birth date on the records before submitting them to Campus Health.
  • The record should have the vaccination name (or type) and the date the vaccination was received.  If you do not have this information there are blood tests that can be done - see links below for more information or contact the Campus Health Clinic. There is also a waiver available in the Campus Health Clinic or by clicking here.
  • There is an Immunization Requirement Form that a medical provider can fill out for the student that includes all of the required immunizations. The form is not required, but it is an option for students and parents. Click here for the form.
  • Mail immunization records and/or waivers to:

Don't Have The Required Immunizations?

The Campus Health clinic can provide some of the immunizations required for a fee during our walk-in hours.

Vaccines are also available free of charge in some situations, or at a lower cost, at the local Oklahoma City/County Health Department. Outside of Oklahoma, most state or city health departments offer clinics to provide low-cost vaccination. Most vaccinations are free at the health department if you are 18 years old or younger, so don't wait to get your vaccinations.

Nursing Student?

Kramer School of Nursing requires immunizations for students enrolled in clinicals. Click here for information on the required immunization for Nursing Students.
