Health Insurance Waiver - For Domestic (U.S. Citizen) Students Only
- Beginning with the 2007-8 Academic Year all full-time domestic undergraduate (enrolled in 12 hours or more) and graduate (enrolled in 9 hours or more) students must indicate one of the following in order to address health insurance: (1) The student is enrolled in a health insurance plan and provides information about the plan in the Online Health Insurance Waiver Form; (2) The student desires to enroll and enrolls in the University-sponsored health insurance plan (Click Here to Enroll); or (3) The student has no health insurance and no desire to enroll in the University-sponsored health insurance plan by filling out the applicable sections of the Online Health Insurance Waiver Form.
- Why is this important?
- Over 30% of the uninsured population in the U.S. is between the ages of 18 and 24, or college age, according to the U.S. Census Bureau.
- It’s important for everyone, including students, to have access to health insurance and health care.
- Primary healthcare services are offered at very low cost to students at the Student Health Clinic on campus. However, if a student needs healthcare off-campus or after-hours in Oklahoma City, health insurance is a necessity in order to avoid potentially large medical bills. Some specialists and doctor's offices will not take patients who do not have health insurance.
- In order to address this important issue, all students need to fill out the short Online Health Insurance Waiver Form before September 7th. The waiver form will need to be filled out each Fall for full-time students.

- Simply fill out all of the applicable information on the Online Health Insurance Waiver form. All of the information about your current heatlh insurance plan can be found on your health insurance card.

Some helpful tips on the Student Health Insurance:
- A claim form is required for each illness or injury. These are available in the Student Health Center or online. Claim forms need to be filled out no later than 30 days after an injury or illness.
- Students must use the Student Health Center for all non-emergency illness or injuries before seeking treatment elsewhere.
- Students need to show their health insurance card to all healthcare providers before seeking treatment. This means you need to carry your health insurance card with you at all times.
- If you receive any bills or letters about medical care or health insurance, bring them to the Student Health Center immediately. If you wait too long, there may be nothing that Student Health can do to assist you.
