Verify that you and/or your parents were not required to file a tax return.  Complete the form and list all income and submit the completed form with any additional documentation to the Financial Aid Office.

•   Affidavit for Reason of Unavailability of Tax Transcript (.pdf)

Per Federal Regulations to process verification the student and/or parent must submit a Tax Return Transcript.  If a Tax Return Transcript is unavailable to you and/or your parent(s) submit the completed form and required additional documentation to the Financial Aid Office for review.

•   Affidavit of Identity and Statement of Educational Purpose (.pdf)

This form may be requested as part of the verification process.  Student must appear in person at the Financial Aid Office with required identification to submit and sign the form.  If student cannot appear in person follow the specific instruction for submitting the form with a Notary’s Certificate of Acknowledgment.

•   Authorization to Release Financial Information (.pdf)

Student can complete form to authorization the Financial Aid office to release financial information to parents and/or spouse.  Student must submit this form in person to the Financial Aid Office.

•   Direct Deposit - Parent Plus (.pdf)

All refunds are made by direct deposit in to personal checking/or savings accounts.  We must have a direct deposit form on file for any Parent that has a Parent Plus Loan.  This will ensure that if there is a refund of the Parent Plus loan the funds will be sent to the correct account.

•   Emergency Advance Application (.pdf)

On the first day of class a student can submit the application to request an advance of any pending refund of excess aid for the current semester.  The amount is limited to $400 for undergraduate and $600 for graduate students.  Student must submit form in person to the Financial Aid Office for review.

•   Parent Resource Letter (.pdf)

Form is requested when parent(s) report unusually low income on the FAFSA.  The information will be used to confirm living arrangements and ensure accuracy of your application.  Student and parent must sign the form and submit to the Financial Aid Office.

•   Request for Additional Bar Exam Registration Expenses (.pdf)

Students may request additional aid to fund bar exam registration fees.  If approved additional Graduate Plus loan can be offered.  Submit completed form with detailed supporting documentation and or receipts to the Financial Aid Office for review.

•   Request for Additional Financial Aid for Education-Related Expenses (.pdf)

If a student has additional education related expenses that result from attending OCU they may request additional aid.  If approved additional Parent Plus loan or Graduate Plus loans can be offered.  Submit completed form with detailed supporting documentation and or receipts to the Financial Aid Office for review.

•   Request for Additional Financial Aid for Study Abroad (.pdf)

Students who want to participate in a Study Abroad program and use financial aid must complete the form. Allow 30 days for application results.  Submit to the Financial Aid Office with all requested documentation.

•   Satisfactory Academic Progress Policy (.pdf)

A student must demonstrate Satisfactory Academic Progress (SAP) to receive financial aid.  The SAP Policy defines the completion rate and grade point average required to remain eligible for financial aid.

This form may be request if you indicated on the FAFSA that either you or your parent(s) paid child support.  Submit the completed form signed by you and your parent to the Financial Aid Office.

•   Verification of Child Support Paid for Independent Students (.pdf)

This form may be request if you indicated on the FAFSA that either you or your spouse paid child support.  Submit the completed form signed by you and/or your spouse to the Financial Aid Office.

•   Verification of SNAP Benefits (.pdf)

This form may be requested as part of the verification process if you indicated on the FAFSA that a member of the household received Food Stamps.  Submit completed form and documentation from the agency that issued the Food Stamps to the Financial Aid Office.  If you are a dependent student both student and parent must sign form. 

•   Verification Worksheet for Dependent Students (.pdf)

Verification is a process to review the accuracy of the information reported on your FAFSA by comparing it with your and your parent(s) Federal income tax information and/or other financial documents. Submit the completed form signed by you and a parent with any additional documentation requested to the Financial Aid Office.

Perkins Exit – Part II (Important)

Once you complete your Perkins Exit Counseling online, please read the Perkins Information Packet.  This packet will provide vital information regarding your Perkins lender, where to send payments, consequences if you default, and your responsibilities regarding the loan. After all documents have been reviewed, sign, date, and return the last page of the packet – Perkins Information Acknowledgement- to the Student Accounts Office.

By completing this requirement, it will finalize your Perkins Exit and the Perkins hold will be released from your Student Account.

If you are entering one of the fields listed below, you may qualify for loan cancellation benefits. For you convenience the cancellation information is available to you at Loan Forgiveness or you can schedule an appointment with the Perkins Loan Counselor by e-mailing finaid@okcu.edu.
