What if I don't have the required immunizations?
The Campus Health clinic can provide some of the immunizations required for a fee. Vaccination fees vary; please call Family Healthcare and Minor Emergency Clinic at OCU at 405-373-2400 for current pricing information.
Vaccines are also available free of charge in some situations, or at a lower cost, at the local Oklahoma City/County Health Department.
Outside of Oklahoma, most state or city health departments offer clinics to provide low-cost vaccination. Most vaccinations are free at the health department if you are 18 years old or younger, so don't wait to get your vaccinations.
Once You've Gotten Vaccinated
- Have your doctor or medical provider complete the OCU Immunization Requirements form — download the PDF and print a copy of it.
- Make a copy of the completed form and return it to Student Health Services in person, by mail, by fax, or using the secure upload link below.
Declining Vaccinations
If you have a medical, moral, or religious reason for not being vaccinated:
- Make an appointment with the campus health clinic by calling 405.373.2400.
- As required by state law, a health care provider will share educational information about the risks and benefits associated with hepatitis B, measles, mumps, rubella (MMR) and meningococcal vaccinations.
- If requesting a medical exemption, the student must have a signed statement from a licensed provider indicating each vaccine that is medically contraindicated. This letter should be provided to the healthcare provider at the clinic at the time of the appointment.
- After review of the educational information about the communicable diseases and the benefits of the vaccines, the student must sign an immunization waiver. If the student is under the age of 18 the parent or legal guardian must also sign the immunization waiver.
If you are less than 18 years old, your parent or guardian must also sign the Immunization waiver form.
